In each case, check off each addressee to whom you are sending the letter. For example, if the mail is being sent to Y. Ya. Chaika, place a ‘check’ next to his duty title.

The text of the letter should be as follows:

From: Surname, given name, patronymic (for example: Ivanov, Ivan Ivanovich) and your full postal address.

Text of the letter: “I, name, fully endorse the Application of attorney Moskalenko K. A. dated October 26th, 2012, protecting the interests of Finogenov et al, and wish to join this claim and insist on the execution of the Court’s decision in this case.

“To this end, I ask that a criminal case be opened, that all necessary investigations mentioned in the Application be carried out, and that those responsible be brought to justice.

“I ask to be acknowledged as a victim in the criminal case, and party to the civil suit.”
